1. What a concierge practice needs from the software

A concierge practice is a prescribing primary care practice with a membership on top. That rules out a non-prescribing plan for almost everyone, and it means memberships and messaging matter more than they do in most clinics.

Need Why it matters in concierge medicine On Aminova
Charting and e-prescribing, including controlled substances You are the member’s physician, not a single-service clinic Prescribing plan
Labs and telehealth Results and video visits are part of the access members pay for Prescribing plan
Membership billing The retainer is the revenue Included: recurring plans, family plans, visit allowances
Secure messaging Direct access is the product Included on every plan
Patient portal Booking, forms, payments, results, refill requests Included
AI scribe Longer visits make more documentation One seat included with Prescribing
Insurance billing Only for a hybrid practice Add-on
Your own app in the app stores A visible member perk Add-on

Our Start-up plan costs less but has no e-prescribing, labs or telehealth, so for a concierge physician who prescribes, Prescribing is the realistic plan and the rest of this post prices it. The full feature comparison across EMRs is in the best EMR for concierge medicine.

2. The year-one bill for a solo concierge physician

Two routes, both with the $250 activation fee, which covers setting up e-prescribing (including controlled substances) and moving your data. The plan itself starts when you go live, not when you open the account.

Route Activation Plan, year one Year one Year two
Prescribing, monthly $250 3 × $99 + 9 × $199 = $2,088 $2,338 12 × $199 = $2,388
Prescribing, annual $250 $1,990 (2 months free) $2,240 $1,990

The $99 intro rate applies to new clinics on the Prescribing plan billed monthly, for the first three months, with the AI scribe seat included; from month four it is $199 a month. Annual billing is $1,990 a year, which works out at about $166 a month and saves $398 against twelve months at $199; it is set up with our team, and the intro rate does not apply to it. A second prescriber is $99 a month, or $990 a year. Your whole team — nurse, medical assistant, front desk, care coordinator — is included at no seat cost, which matters in a practice where the physician is the only prescriber.

3. What the software costs per member, per month

Divide the software bill by the panel and it becomes a small line in a concierge budget. The table uses the standing rate — $199 a month on monthly billing, or $1,990 a year on annual — for a solo physician on the Prescribing plan with no add-ons. The panel sizes are illustrations; put in your own.

Members on the panel Monthly billing ($199/mo) Annual billing ($1,990/yr) Year one, monthly ($2,338)
100 $1.99 per member per month $1.66 per member per month $1.95 per member per month
300 $0.66 $0.55 $0.65
600 $0.33 $0.28 $0.32

The add-ons change those figures less than you might expect. The white-label app at $79 a month adds $0.79 per member per month at 100 members, $0.26 at 300 and $0.13 at 600, plus $750 once. An extra AI scribe seat at $89 a month for a second clinician adds $0.89, $0.30 and $0.15 respectively. Whatever your retainer is, the software per member is likely to be a very small fraction of it; the bigger risk is a product that charges per staff login or takes a percentage of revenue, both of which grow with the practice. We do neither: there is no percentage of your revenue, and staff are unlimited.

4. The add-ons a concierge practice actually needs

Most concierge practices need none of the add-ons on day one. Three are worth knowing about, and each depends on how you run the practice.

Add-on Published price When a concierge practice needs it
Insurance billing Priced with you, based on claim volume Only if you run a hybrid practice that bills insurance for covered visits. Superbills are included on every plan without it
White-label patient app $750 once, then $79/mo When you want members to find your practice’s own app in both app stores, under your name and logo — a visible perk of membership
Extra AI scribe seats $89/mo per seat Only for each additional clinician who sees patients; one seat comes with Prescribing
Website, built and run by us $99/mo Only if you do not already have a site that works

On the scribe, the fair-use allowance is 100 scribed hours per seat per month, then $0.50 an hour; concierge visits run long, but a solo physician is unlikely to approach it. On insurance, the add-on handles real-time eligibility, claims built from the encounter and remittance posting, but payer enrollment takes weeks and your clearinghouse may have its own costs, so a hybrid practice should budget time as well as money. On the app, plan on a few weeks for store review, which no vendor controls; we handle submission. More on what a branded app involves is in the white-label patient app guide.

5. How to compare concierge EMR quotes

Concierge practices get caught by different things than other clinics, so ask these questions alongside the price.

Ask Why it matters for a concierge practice
Is membership billing included, or a separate subscription? Some stacks need a second product to bill the retainer, with its own fee and its own login
Does the price rise with each staff login? A nurse and a care coordinator should not raise the software bill
Is any of it a percentage of revenue or of membership fees? A percentage grows with every member you add
Can a family plan bill once while keeping separate charts? Households are common in concierge panels
Is there a contract term, and what does leaving cost? A small panel cannot carry a year of the wrong software
Will they sign a BAA? Any vendor holding member records must

The general method is in how to evaluate clinic software, and the pricing structures themselves are explained in clinic software pricing models. On exit, we publish a free full data export at any time and plans are month-to-month; what it costs to leave your EHR covers what to ask everyone else. On BAAs, see which vendors sign a BAA. If you are still at the planning stage, how to start a concierge medicine practice puts the software in the order it is needed, and concierge membership billing covers setting up the retainer itself.

6. Frequently asked questions

How much does an EMR cost for a concierge practice?
On Aminova, a solo concierge physician on the Prescribing plan pays $2,338 in year one on monthly billing ($250 activation, $99 a month for three months, then $199) or $2,240 on annual billing ($250 plus $1,990). Year two is $2,388 monthly or $1,990 annual.

What does the EMR cost per member?
At the $199 standing rate, about $1.99 per member per month at 100 members, $0.66 at 300 and $0.33 at 600. On annual billing it is about $1.66, $0.55 and $0.28.

Does a concierge practice need insurance billing?
Only a hybrid practice that bills insurance for covered visits. A retainer-only practice can give members superbills, which are included on every Aminova plan. The insurance add-on is priced with you, based on claim volume.

Do I pay for my nurse and front desk?
Not on Aminova. Pricing is per prescriber and staff are unlimited, so a nurse, medical assistant or care coordinator does not add a seat.

Can I start a concierge practice on the free Start-up plan?
Only if you are not prescribing yet. Start-up is $0 for the first three months as a founding offer limited to the first 100 clinics, then $99 a month, but it has no e-prescribing, labs or telehealth.
